Release of Liability, Assumption of Risk, Medical Authorization, and Conduct Agreement
I, the undersigned parent/legal guardian of the minor participant named below (“Camper”), understand and agree that participation in Celt Summer Camp and any related activities, whether on or off campus, is voluntary and may involve known and unknown risks, including but not limited to physical injury, illness, allergic reaction, emotional distress, property loss or damage, transportation-related incidents, and, in rare circumstances, serious injury, disability, or death.
In consideration for Camper being permitted to participate in Celt Summer Camp, I, on behalf of myself and to the fullest extent permitted by law on behalf of Camper, hereby acknowledge and agree as follows:
1. Assumption of Risk
I understand that camp activities may include athletic, recreational, classroom, outdoor, group, and supervised free-time activities, and that such activities carry inherent risks that cannot be completely eliminated without fundamentally changing the nature of the program. I knowingly and voluntarily assume all risks, both known and unknown, arising from or related to Camper’s attendance at and participation in Celt Summer Camp, including risks arising from ordinary negligence of Crespi Carmelite High School and its officers, directors, trustees, administrators, employees, agents, coaches, volunteers, contractors, and representatives (“Released Parties”), to the fullest extent permitted by law.
2. Release and Waiver of Claims
To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue the Released Parties for any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses of any kind arising out of or related to Camper’s participation in Celt Summer Camp, including claims for personal injury, illness, death, or property damage, if caused by ordinary negligence. This release is intended to be as broad and inclusive as permitted under California law.
This release does not apply to claims arising from gross negligence, reckless misconduct, or willful misconduct to the extent such claims cannot be waived under applicable law.
3. Medical Authorization and Emergency Care
If I cannot be reached in an emergency, I authorize Crespi Carmelite High School and its personnel to secure and consent to emergency medical, dental, hospital, surgical, ambulance, or other health care for Camper as deemed reasonably necessary by medical professionals or emergency responders. I understand this authorization is intended to avoid delay in treatment.
I agree that I am solely responsible for all medical and related expenses incurred on behalf of Camper, including ambulance, hospital, physician, pharmacy, and follow-up care costs. I further understand that Crespi Carmelite High School does not assume any duty to provide medical insurance for Camper.
4. Health Information / Fitness to Participate
I represent that Camper is physically and emotionally able to participate safely in camp activities and that I have disclosed to Crespi, in writing, any medical condition, allergy, injury, disability, behavioral concern, dietary restriction, medication need, or other circumstance that may affect Camper’s safe participation or the safety of others. I understand that failure to provide complete and accurate information may increase risk to Camper and others and may result in Camper’s dismissal from camp without refund.
5. Medication
I understand that Crespi Carmelite High School will only administer medication in accordance with its camp policies and any required forms or physician instructions. I agree that Camper will not bring, possess, share, or use any medication except as expressly authorized by Crespi in advance and in compliance with camp procedures.
6. Conduct / Discipline / Dismissal
I understand that Celt Summer Camp is expected to maintain a safe, respectful, and orderly environment. Camper agrees to follow all camp rules, directions, and behavioral expectations at all times.
The following conduct may result in disciplinary action, immediate parent contact, suspension from activities, or dismissal from camp, with or without refund, at Crespi’s sole discretion:
copying homework, assignments, classwork, projects, quizzes, or tests;
inappropriate language;
disrespectful, disruptive, bullying, aggressive, or unsafe behavior;
harassment of any kind;
damage to property;
possession of prohibited items; or
refusal to follow staff instructions or camp rules.
I understand that Crespi may require a separate behavior contract and that violation of camp expectations may result in Camper being removed from camp.
7. Property Damage / Reimbursement
I agree to be financially responsible for any loss of, or damage to, property caused by Camper, whether to Crespi property, facilities, equipment, vehicles, or the property of others, to the extent caused by Camper’s intentional misconduct, rule violation, or negligent acts.
8. Indemnification for Parent/Guardian Breach or Camper Misconduct
To the fullest extent permitted by law, I agree to indemnify and hold harmless the Released Parties from third-party claims, losses, damages, liabilities, or expenses, including reasonable attorneys’ fees, arising out of:
(a) my breach of this Agreement;
(b) my failure to disclose relevant medical, behavioral, or safety information; or
(c) Camper’s intentional misconduct or damage to persons or property.
9. Photography / Communications / Transportation
I understand that camp activities may involve movement between areas of campus and, if disclosed by camp materials, off-site activities or transportation. I consent to Camper’s participation in such supervised activities as part of the camp program.

10. No Guarantees / Program Changes
I understand that Crespi reserves the right to modify camp activities, schedules, staffing, groupings, locations, and supervision arrangements as reasonably necessary for safety, operations, enrollment, weather, facility issues, or other circumstances.
11. Severability / Governing Law
If any portion of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by and construed in accordance with the laws of the State of California.
12. Acknowledgment
I have carefully read this Agreement, fully understand its contents, and sign it voluntarily. I understand that I am giving up substantial legal rights on behalf of myself and, to the fullest extent permitted by law, on behalf of Camper.
